Mmm an eyepatch there's nothing wrong with his eye, he just wears it during the day and then switches it at night so it's easier to see- thus the other pupil is more dilated.

And he smokes (-cough-anddrinks-cough-) but hey I'm not trying to make Belial a role-model here.
